脳腫瘍の放射線治療とCerebrovasculopathy

Therapeutic Irradiation for Brain Tumor and Cerebrovasculopathy

I．はじめに

　近年，悪性脳腫瘍に対して化学療法剤と放射線照射による強力な治療が行われるようになり，著しい効果を上げている．しかし，一方では，放射線照射による種々の副作用や二次的障害が報告されるようになり，なかでも，白質脳症や脳血管障害などの重大な中枢神経系の障害は患者の一生を左右する点で重大な問題として論議されている．

　1967年，Leeら^8）が，脳腫瘍の放射線治療後に脳血管障害を発生したradiation vasculopathyと考えられる小児例を報告して以来，同様の症例が漸次報告されてきている．

The first case was a 5-year-old girl treated with a tot-al of 6000 rads after total removal of a left temporopa-rietal tumor extending into the basal ganglia. About 4 years after completion of the radiation therapy, she showed left hemiplegia and deterioration in her level of consciousness. A plain CT scan showed calcification in the region of the bilateral basal ganglia and low den-sity area in the right fronto-parietal region. An en-hanced CT revealed gyral enhancements in the pre-and postcentral gyrus of the right hemisphere.
