I．はじめに

　椎骨脳底動脈の頭蓋内閉塞あるいは狭窄症に対する血行再建術として，浅側頭動脈と上小脳動脈の吻合^3,4）と大伏在静脈片を用いた外頸動脈と後大脳動脈との吻合^11）がある．これらの術式はそれぞれに一長一短があり，現時点で優劣を決めることはできない．しかし，吻合部を介する血流量についてみれば，Sundtらの静脈移植片を用いる吻合術は術直後より血流量が多く明らかに優れている．私どもは脳幹部虚血症状が進行性に悪化した頭蓋内椎骨動脈の一側閉塞，他側狭窄例に脳幹部虚血の速やかな回復を期待して大伏在静脈片を用いた外頸動脈-後大脳動脈吻合術を行い，術後，症状の進行は止まり，さらに，一部症状の改善を得たので手術手技の詳細を加えて報告する．

A successful case undergoing the ECA-PCA bypass operation with the use of an interposition saphenous venous graft for vertebrobasilar progressing stroke was reported and details of the operative techniques were described.

A 40-year-old man was admitted because of con-fused mental state following sudden onset of headache, vomiting, vertigo, and ataxic gait. Neurological examinations revealed he was confused and restless, and left-sided Weber's syndrome, bulbar palsy and dysphasia were noticed. CT scan showed multiple small low density areas with no enhancement scattering in both occipital lobes and cerebellar hemispheres.
