総頸動脈狭窄例に対する大伏在静脈を用いた鎖骨下動脈—外頸動脈バイパス術の1例

Subclavian Artery to External Carotid Artery Bypass with Saphenous Vein Graft for the Treatment of Common Carotid Artery Stenosis: A case report

I．はじめに

　総頸動脈閉塞性病変に対する外科的治療法^4,7,9）は，その位置と広がり，解剖学的状況に応じて種々の方法が選択される．血栓内膜切除術を中心とした直達手術に加え，顕微鏡を用いた各種バイパス手術の頻度も増加しつつある．最近，われわれは総頸動脈起始部狭窄例に対し大伏在静脈移植による鎖骨下動脈外頸動脈バイパス術を行い，良好な結果を得たので報告する．

A case of TIAs due to proximal common carotid artery stenosis which was successfully treated withautogenous saphenous vein graft between the subcla-vian artery and the external carotid artery is presented.

A 57-year-old, right handed female was admitted to our hospital for the treatment of left common carotid artery stenosis which was pointed out at a local hospital. She had a 7-years' history of repeated tran-sient right hemiparesis and/or left amaurosis fugax. No neurological deficit was revealed on admission.
