多発性脳動脈瘤を合併した急性特発性硬膜下血腫の1症例

Acute Spontaneous Subdural Hematoma associated with Multiple Aneurysms:A case report

I，はじめに

　急性硬膜下血腫は，多くの場合，重症頭部外傷にみられ，脳挫傷に伴う血管損傷がその成因であることが多い．また，脳動脈瘤の破裂や動静脈奇形からの出血による硬膜下血腫の報告も少なからず散見される^{2,3,7,11,12）}．一方，このような従来からの報告の外に，非外傷性で血管奇形などもみられない，いわゆる急性特発性硬膜下血腫の存在も知られている^{1,4-6,8-10,13,14,17,18）}．

　今回われわれは，突然の頭痛で発症した非外傷性急陸硬膜下血腫例において，脳血管撮影で多発性脳動脈瘤が認められたため，その成因の考察に苦慮したが，手術的に急性特発性硬膜下血腫と診断した症例を経験したので，若干の文献的考察を加えて報告する．

A case of acute spontaneous subdural hematoma associated with three aneurysms is reported.

On March 12, 1984, a 47-year-old woman experi-enced the sudden onset of severe headache over the bilateral frontal region and vomiting. Three hours later, she was transfered to our hospital by ambu-lance car because of continuous headache and vom-iting. She had no history of head trauma. She had been medicated hypertension for five years. On ad-mission she suffered from headache and nausea. But there was no clinical sign in physical and neurological examinations.
