Mixed Pial-Dural Arteriovenous Malformation: Case report

I．はじめに

　Newton^10）らによれば，頭蓋内動静脈奇形（以下AVMと略す）は脳血管撮影上認められるfeederの種類により，Pure pial AVM，mixed pial-dural AVM, pure dural AVMの3つに大別されるが，mixed pial-duralAVMは頻度も低く，その報告は少ない^{2-5，10-14,16）}．また，その発生機序に関しては定説がなく，疑問点も多い．

　今回われわれは，内頸動脈の皮質枝および外頸動脈の中硬膜枝をfeederとする天幕上，頭頂部に発生したmixed pial-dural AVMの1例を経験したが，本症例はmixed pial-dural AVMの発生機序，特にrete mirabile anasto-mose^1,9,18）などの生理的な側副血行路のAVMへの関与という点で極めて示唆に富むと考え，その臨床経過を報告し，文献的考察を加える．

The authors report a case of mixed pial-duralarteriovenous malformation (mixed pial-dural AVM),which had feeders from both cortical branches of theinternal carotid artery and middle meningeal arteryof the external carotid artery. A clinical course ofthis case is presented and literatures are reviewed.Moreover the authors stress the rete mirabile anasto-moses as an important factor in the genesis of themixed pial-dural AVM.

The patient, 30-year-old female, suddenly sufferedfrom a severe headache on December 31, followed bygradual deterioration of consciousness.
