I．はじめに

　くも膜嚢胞は比較的まれな疾患であるが，近年CTscanの普及により，頭痛やけいれん発作または頭部外傷などの検査の際に偶然発見されることが多くなった．くも膜嚢胞は，くも膜下腔の存在する所であればどこでも発生可能であるが，中頭蓋窩のものが多い．

　この中頭蓋窩のくも膜嚢胞に慢性硬膜下血腫を合併することはしばしば報告されている．これは偶発的合併でないことはほぼ明らかであるが，その発現機序については充分な説明がなされていない，最近，われわれは中頭蓋窩くも膜嚢胞に慢性硬膜下血腫を合併した5症例を経験したので，症例の分析により，くも膜嚢胞における硬膜下血腫の発生機序について若干の考察を加えて報告する．

A significant number of cases of chronic subduralhematoma associated with middle fossa arachnoid cysthas been reported in literature, but sufficiently tenableexplanation for co-occurrence of both lesions has notyet proposed. In this study, authors try to elucidatemechanisms involved in development of chronic sub-dural hematoma and arachnoid cyst in the same patient.

Eighteen cases with arachnoid cyst in the middlefossa were diagnosed by CT scan during last 5 yearsin our institute. Among these, five patients hadchronic subdural hematoma additionally to theirmiddle fossa arachnoid cyst.
