I．緒言

　現在，水頭症に対する治療法として脳室心房短絡術（以後V-A shuntと略す）および脳室腹腔短絡術（V-Pshuntと略す）が広く行われ，また難治性の硬膜下腔液貯留に対して硬膜下腟腹腔短絡術（S-P shuntと略す）がよく用いられている．これらのshunt手術は簡便であり，極めて有効な手段ではあるが，一方，shunt ca-theterの閉塞および位置移動などの合併症が頻々と経験される．このため，shunt catheterの材質の改良および手術術式の工夫など本術式にはなお残された問題がある．

　今回，著者らは両側慢性硬膜下血腫後の硬膜下水腫症例に対してS-P shuntを施行したところ，一側の全shunt systemが硬膜下腟内へ迷入した症例を経験したので，本例のshunt術後の合併症の機序について若干の考察を加えて報告する．

A ten-month-old baby was admitted to our clinic withthe diagnosis of bilateral chronic subdural hematomas. Hehad complained of general convulsion during the past 3months. Evacuation of hematomas was performed on bothsides through burr holes in the parietal region. Severalweeks following the evacuation of hematomas, subduro-peritoneal (S-P) shunting operation was further performedbilaterally for the collection of fluid in the subdural spaces,using a "Pudenz" shunting catheter with a reservoir. Thecatheter was fixed at the connector of the reservoir and theperitoneal catheter by several silk sutures to the epicranium.
