大動脈炎症候群に伴った前交通動脈瘤の1手術治験例

Aneurysm of Anterior Communicating Artery Associated with Aortitis Syndrome:A treated case by aneurysmal neck clipping

I．はじめに

　大動脈炎症候群とは，大動脈およびそれに近接した基幹動脈が後天的な慢性非特異性炎症により，血管内腔の狭窄や閉塞を来たしたために生ずる諸症状を総称したものであり，"脈なし病"^13,14,18）とか"高安病"^19）と呼ばれている．本症候群は圧倒的に女性に多く（約96％），しかもその70％が10代，20代の若い女性に発生する^13,14）．一方，脳動脈瘤と本症候群の合併の報告は，渉猟しえた限りでは7例であり，救命3例^1,5），死亡4例^5,8,16）である，直達手術により救命された症例は本症例を含めて2例^5）のみであり，その発生部位は，4例が椎骨-脳底動脈系，2例が内頸-後交通動脈分岐部，前交通動脈瘤は本症例のみであるため稀有な症例と考え，症例報告とともに，脳動脈瘤の発生成因，動脈瘤直達手術の注意事項などについて若干の考察を加える．

A rare case of the cerebral aneurysm associated withaortitis syndrome was reported. It seems to be the firstcase in which neck clipping was successfully performed foraneurysm of the anterior communicating artery. Thepatient was a 48-year-old female afflicted with pulselessdisease. She was admitted to Iwate Central Hospital withsevere headache, vomiting and unconsciousness on November16, 1980. Her radial pulse was faint on the right side. Theleft carotid angiography revealed an aneurysm in the anteriorcommunicating artery, and neck clipping of the aneurysm wasperformed 17 days after the onset.
