Ⅰ．はじめに

　天幕切痕部腫瘍は比較的まれなものであるが，腫瘍がextra-axialにあるので外科的摘出が可能であることが多い．当領域腫瘍のX線診断については既にCastellano^3），Taveras^9），Zingesser^12）等のすぐれた研究があるが，その解剖学的位置関係が複雑なため，なおX線診断が困難であることが少なくない．殊に脳幹部のintra-axial腫瘍との鑑別が問題である．

　従来，当該部腫瘍は脳幹部に対する位置関係からanterior, posteriorおよびlateral typeに大別された^9）．posteriorおよびlateral typeのうちには天幕の上下にわたる（dumbbell type腫瘍のあることは知られており，文献上報告例の大多数がmeningioma^{1，3，7，8，9，12）}で，一部がneurinomaであった．Carrefour falcotentorial meningioma^{3）より引用}の多くは天幕上下にわたるもので，Cushing, Eisenhardtの言葉を借りれば　meningioma perforating of the tentoriumであった．かくの如く当該部に発生した腫瘍としてmeningiomaは古くから知られているが，meningiomaあるいはneurinoma以外の腫瘍については，調べた範囲においてその報告はみられず，まれなもののようである．

It is well noted about the dumbbell type of meningioma arising in the incisura of the tentorium. However, another histological feature of this type of tumor is rarely reported.

We had, recently, two cases of dumbbell type tumor in the incisural region, one ependymoma and the other epidermoid cyst. The significance of the vertebral angiogram was especially noted in the diagnosis of these tumors.

Case 1. H. K., 5-year-old girl was admitted to our hospital on April 10, 1968. She complained of progressive headache, vomitting and unsteadiness of her walk.
