I.緒言
Moyamoya病31)における初期変化は,両側内頸動脈終末部付近(carotid fork)を中心とする狭窄性変化であり,その狭窄の緩徐な進行に伴って脳底部にMoyamoya血管が出現してくる.この脳底部のMoyamoya血管については,これまで種々論議12,15,15,29)があったが,現在では狭窄性変化に伴って発達した脳底部穿通動脈を母体とする側副血行路10,11,19,28,31-34)であると考えられている.しかし,本疾患のinitial causeであるcarotid forkを中心とした狭窄性変化の原因については,いまだその解決の糸口さえもつかめていない.
本疾患におけるcarotid forkを中心とする脳主幹動脈の病理所見1,6,30)は,内膜の細胞性もしくは線維性肥厚,内弾性板の屈曲蛇行,断裂,重層化そして中膜の菲薄化がその主たるものであり,これらの所見は多発性動脈炎16,39)や川崎病2,37)などの免疫反応由来と思われる血管炎の陳旧性病変の病理所見と類似している.
Initial pathological finding in Moyamoya disease is stenoticchange at the carotid fork due to some unknown causes.Many discussions have been made on the causal genesis ofthis change at the carotid fork, but there have been no proofon them nor any experimental study. Based on our studyof 100 cases of this disease, we have found that many patientsexperienced chronic inflammation above the neck, i.e. chronictonsillitis, neck phlegmon, and that pathological arterialchanges in autopsy cases are similar to those of angitis dueto immunological reaction such as panarteritis.
