I．はじめに

　近年，神経耳科的検査法と神経放射線学の進歩およびCT scanの導入により，小脳橋角部腫瘍の早期診断が可能となっているが，なおかなりの大きさになるまで診断されずにいるのが現状である．その手術法も，従来より行われているsuboccipitalapproachに加え，microscopeの導入以来，middle fossa approachやtranslabyrin-thine approachなども行われるようになっている．

　今回われわれは，脳幹部，小脳および脳神経を温存するという目的で，後頭蓋窩疾患，特に小脳橋角部腫瘍に対して，必要に応じてsuboccipital approachも併用しうるsubtemporal transtentorial approachを行ってきたので，その利点と同時に，問題点についても，併せて検討し報告する．

The authers operated on seven posterior fossa lesionsusing subtemporal transtentorial (STT) approach. Theyconsisted of five CP-angle tumors, one vertebral arteryunion aneurysm and one trigeminal neuralgia for posteriorfossa rhizotomy.

This paper is concerned with five cases of CP-angle tumorswhose diameter over 3cm. In two cases out of the five,combined use of suboccipital approach was needed. Opera-tive procedures were presented, then the results and thecomplications were discussed in this paper.

With STT approach, total removal of the tumor waspossible in all cases.
