I．はじめに

　頭部外傷急性期の剖検時に，mass lesionを有しない一次性の脳幹部損傷（著者らの用いる脳幹部とは，corpus striatum，diencephalon，mesencephalon，Ponsおよびmedulla oblongataまでを総括する広義の脳幹部を指す^19））が存在する．事実は，既に諸家の述べるとおりである^{1,6,20,21,24,33,34）}．しかしながらCT scanが頭部外傷の診断に広く用いられ，絶大な威力を発揮するようになった昨今でも，これにより確かめられた脳幹部損傷の報告は極めて少なく，その詳細な解析はほとんどなされていない．最近著者らは，CT scan上でhigh densityとして確認しえた6例の外傷性脳幹部損傷を経験し，これらのCT所見および発生機序，更に予後との関係を中心に検討したので報告する．

Nowadays computerized tomographic (CT) scan has been widely used for the diagnosis and management of patients with severe head injury because of its safety, rapidity, and ability to allow visualization of the entire intracranial contents.

But, to our knowledge, there has not been many series, dealing with the occurrence of primary brainstem hemorrhage in blunt head trauma identified by CT scan. According to the recent series the incidence of primary brainstem hemorrhage is about 20% among patients showing "primary brainstem injury" clinically.
