Ⅰ．はじめに

　頭部外傷による脳内血腫の発生頻度は少ない．しかし最近CTの導入により，これら血腫の有無，部位が正確に診断され，更に脳室穿破についての情報も容易に得られるようになった．

　われわれは最近，大酒家で飲酒後階段より転落し脳内血腫の発生をきたし，しかもこれらが脳室内へ穿破した3例を経験した．このうち2例はCT上高血圧性脳出血の部位と一致しており，これらの血腫発生の機序および予後について若干の文献的考察を加え報告したい．

Three heavy drinkers, the patients fell down stairs whime inebriated and were hit on their heads. Utilizing CT, we found that the hematomas in all three cases were accompanied by ventricular hemorrhage which, in two of the cases, were similar to hypertensive intracere-bral hematoma in the basal ganglia. None of the three had a history of hypertension. We are of the opinion that the vulnerabihty of cerebral arteries, especially in the basan ganglia, of such heavy drinkers added to the traumatic force and led to the pathogenic bleeding.
