I．はじめに

　側頭葉深部やテント切痕部の病変の手術の際に大切なことは，いかにして適切なアプローチおよび脳縮小を行い，十分な視野を得るかという点である．テント・テント切痕部，錐体部，小脳橋角部の腫瘍や後頭蓋窩の前上方部の動脈瘤等，種々の病変に対するsubtemporal transtentorial approachの報告は多くみられるが，そのpitfallsとしての側頭葉損傷に関する報告は極めて少ない．著者らは最近経験した6例のsubtemporal transtentorial aproachを行った症例について，側頭葉損傷をみた例とみなかった例を比較検討し，脳損傷を起こす要因について考察を加えたので報告する．

It is essential to make a proper approach to the lesion site and retract brain, thereby securing an adequate field of vision, upon performing operations on patients for lesions of deep temporal lobe and tentorium. Although subtemporal transtentorial approaches to tumors in the tentorium, pyramis and cerebellopontine angle have been described by a number of workers, further studies have to be made on temporal lobe damage as a pitfall since only a few reports have been published to date.
