定位脳手術法により脳内異物（弾丸片）を剔出した1例

A Case of Successful Removal of a Deep-seated Bullet in the Brain by Stereotaxic Approach

Ⅰ．緒言

　従来より脳の深部に存在する異物の剔出は，脳血管写あるいは立体撮影などのレ線学的補助検査を行って，そのおよその位置を知り，開頭術を行って脳の深部でその異物を探しあてて剔出する方法が一般に行われてきた．このような手術法では脳内で異物を探す操作により異物近辺の健常脳組織を不必要に損傷する危険があり，手術侵襲も大きくならざるをえない．したがって，その異物がさしたる神経学的障害を惹起していなければ，むしろ剔出は断念した方がよいとする意見すらみられる．しかし，その異物への安全かつ確実な到達路が決定でき，手術中具体的にその路を知ることができれば，異物剔出操作による脳の副損傷は最小限度となり，かつ手術も簡単となるため，このような異物剔出手術の適応はかなり拡大できると考えられる．

　さて最近われわれは右後頭葉深部に18年間残遺していた弾丸片を定位脳手術により到達路を定め，手術用顕微鏡下にその到達路をたどり，安全に弾丸片を剔出しえた1例を経験したので報告する．

A bullet, which was located deeply in the right occipital lobe of the brain for a 18 years long, was removed by utilization of stereotaxic approach and dissection under operative microscope successfully.

The patient, 31 years old, male, was admitted to our service with episodes of convulsive attacks on February 1, 1978. He had an accident that a bullet penetrated into his brain through his right frontal region by an accidental firing of his own-made gun 18 years ago. There were no neurological signs and symptoms until five years prior to admission, when he suffered from a generalized convulsion in 1973.
