Ⅰ．はじめに

　通常の脳動脈瘤がくも膜下出血で発症するのに対し，巨大脳動脈瘤は出血をきたす事は比較的少なく，頭蓋内占拠物体として周囲の脳組織神経を圧迫して次第に症状を塁してくる事が多い，われわれも最近，両側視力，視野障害で発症した巨大内頸動脈瘤を経験し，最終的には直達手術を行い，一側視力を温存する事が出来た1症例を経験したので臨床経過を追いその問題点をあげ，診断と治療について考察を加える．

The patient, a 60-year-old female, was being followed by an ophthalmologist for 5 to 6 years because of temporal hemianopsia of her right eye. In March 1976 she lost sight in her right eye and at the same time became aware of decreased visual acuity and temporal hemianopsia in her left eye. From about this time she became to be sensitive towards cold and suffered from easy fatigability. She visited The Department of Ophthalmology Hiroshima University Hospital in July 1976, and was referred to The Department of Neurosurgery under the suspicion of parasellar tumor.
