I．はじめに

　きわめて稀な幼児第3脳室皮様嚢腫の症例であるが，診療初期の治療方針が不適当であったために根治手術が遅れた．更に術後は感染との戦いで，1年9カ月の経過で不幸の転帰をとった症例である．結果ははなはだ不本意なこととなったが，われわれにとっては実に教訓の多い症例と思うので，紙面の許す限り詳細な経過を述べてみたい．

A 2 1/2 year-old boy suffered from about 1 month durating rather acutely increased consciousness distur-bance with recurrent vomiting was admitted on May 28, 1975. On admission his consciousness was stupor and increased intracranial pressure, fixed pupil, slightly pyramidal stimulation on both sides were proved in neurological examination. A mass in the third ventricle was revealed accurately by conray ventriculography. He was operated on June 5, 1975 through the right tra-ns-ventricular approach, whereas unfortunately no tumor was found in the third ventricle because of inadequate look for the ventricle.
