Epidermoid of the Third Ventricle

Ⅰ．はじめに

　第3脳室腫瘍は比較的稀であるが，その発生部位と腫瘍の性質を手術前に正確に診断することは必ずしも容易ではない．またこれまでの手術成績も悪く，手術方法についても議論の多いところである．

　私どもは最近経験した第3脳室epidermoidの症例について，補助検査所見より腫瘍の発生部位と種類を術前にいかに診断し，次にいかなる理由で側脳室前角経由の手術法を選択したかについて述べ，諸賢の御批判を仰ぎたいと思う．

The patient was a 13-years-old boy who had become obese progressively and his school record deteriorated for past four and a half years. On admission. his neurological abnormalities were limited to impairment of upward gaze associated with diplopia, right hand tremor and minimal right hemiparesis, His endocrinological examinations were normal. Plain skull film and tomogram showed no pathological findings. CT scan revealed a large ro-und low density mass outlined by curvilinear high den-sity in the third ventricle and symmetrical enlargement of the lateral ventricles.
