脳膿瘍に関する研究（第2報）—被膜外全摘出術の適応と限界

Abscess of the Brain (2nd Report):Special Reference to Surgical Indication of Extracapsular Excision of Brain Abscess

Ⅰ．緒言

　脳膿瘍の外科的治療に関しては，古くから多くの議論がある．脳膿瘍の治療成績が抗生物質の導入前後で格段の差があることは，今や誰も疑う者はいない^{1，2，5，10，11，13，16，19，21）}．しかしながら，近年諸家の報告にみられるように，多種多様の抗生物質が存在するにもかかわらず，脳膿瘍の治療成績は30-40％という高い死亡率を示し，必ずしも満足すべきものではない^{4，6，9，19，26）}．この事実は，脳膿瘍の治療方法，特に手術方法および手術時期に，まだ多くの問題点があることを示唆しているものと考えられる．著者らは，脳膿瘍に関して一連の研究を行ってきたが，今回，当教室で行われた脳膿瘍の手術成績を検討し，治療方法いかんによってmortality, morbidityが大いに異なることを見い出した．そこで，脳膿瘍の外科的治療特に被膜外全摘出術の適応と限界について，自験例を中心に若干の知見を述べてみたい（Fig．1）．

Brain abscess persists as a serious diagnostic problem and critical therapeutic challenge since pre-antibiotic era.

After antibiotic agents has been introduced, abscess of the brain seems to be a surgical curable intracranial suppurative disease, although surgical mortality and morbidity due to brain abscess are still distressingly high. Recently, the incidence of brain abscess are gradually increasing.

In this present situation, it is necessary to reevaluate the previous method and surgical results of brain abscess.
