Ⅰ．はじめに

　現在，脳動脈瘤に関してはその外科的治療方針もほぼ確立され，優秀な手術成績も報告されている．また，脳血管撮影の普及とともに斜位撮影法，頭蓋底撮影法などを行なうことにより脳動脈瘤の発見率もさらに高くなりつつある．

　しかしながら脳血管写上しばしば認められる動脈瘤様の小膨隆像がしだいに大きさを増しはっきりとした脳動脈瘤へと成長した過程を追求し得た症例の報告は少なく，このような小膨隆像の運命に関する報告もまた少ない．

We have reported the case of a 65-year-old woman, in whom very small arterial sacculation of the left internal carotid-posterior communicating artery junction had enlarged radiographically during eight years. In this case, carotid ligation contralateral to the small arterial sacculation and hypertension (so called hemodynamic stress) may have predisposed the enlargement of this sacculation.

Small sacculation may be minute aneurysm or junctional dilatation, and differentiation of these lesions often requires special angiographic procedures.
