Ⅰ．はじめに

　外傷性偽動脈瘤が頭蓋内血腫形成の原因となる頻度はかなり少なく，更にそれが，中硬膜動脈偽動脈瘤の破裂に起因したとする報告は現在まで約30例を数えるに過ぎない．また，この外傷性中硬膜動脈偽動脈瘤は外傷急性期での血管撮影では造影され難く，症状の発現が亜急性期ないしは慢性期における急激な頭蓋内出血の形を取り，放置すれば致命的ともなり得るなど，臨床上いくつかの問題点を持つ．

　一方，外傷性中硬膜動静脈瘻に関しても外国，本邦を合わせて約30例とその報告は決して多くはない．しかもそれら疾患によって起こる頭蓋内血腫の大部分は硬膜外あるいは硬膜下血腫であり，脳内血腫の発生を見たとする報告は数例に過ぎない．今回我々は頭部外傷後，中硬膜動脈偽動脈瘤と中硬膜動静脈瘻を併発し，更に右側頭葉内に脳内血腫を形成した興味ある1例を経験したのでここに報告するとともに，若干の文献的考察を加えたい．

A case of traumatic false aneurysm associated witharteriovenous fistula involving the middle meningeal artery was reported with a review of the world literature on these specific entities. The present case reported here appears to be unique because of the combination of false aneurysm with arteriovenous fistula, and because of the association of intracerebral hematoma.

The combination of traumatic false aneurysm with arteriovenous fistula of middle meningeal artery is relatively uncommon, and only one report has been observed in the world literature.
