We have reported the case of a 65-year-old woman, in whom very small arterial sacculation of the left internal carotid-posterior communicating artery junction had enlarged radiographically during eight years. In this case, carotid ligation contralateral to the small arterial sacculation and hypertension (so called hemodynamic stress) may have predisposed the enlargement of this sacculation.

Small sacculation may be minute aneurysm or junctional dilatation, and differentiation of these lesions often requires special angiographic procedures.
