I．はじめに

　頭部外傷に続発する動静脈瘻の好発部位は，内頸動脈海綿静脈洞部であり^1），中硬膜動脈領域^4），および浅側頭動脈領域^11）の報告も散見される．しかしながら，外傷により眼窩内に発生した動静脈瘻の報告は極めて少なく，われわれが検索し得た限りでは過去2例の報告を見るのみである^5,9）．そして，何れの報告も穿通性外傷による直接的血管損傷によるものである．今回われわれは，非穿通性頭部外傷に起因すると思われる，きわめて稀な眼窩内動静脈瘻の1例を経験したので，若干の文献的考察を加えて報告する．

Traumatic intraorbital arteriovenous fistula is rare and only 2 cases have been reported. The authors re-port the case of a 70-year-old man with consciousness disturbance after non-penetrating head injury. CT scan showed diffuse subarachnoid hemorrhage in the basal cistern, and angiography on the next day revealed opthalmic artery-superior ophthalmic vein fistula in the right orbit. During the clinical course of this lesion, there was mild chemosis of the right eye, without palsy of extraocular muscles. Both papilledema and bruit were absent. Conservative treatment was chosen.
