Ⅰ．はじめに

　直接頭蓋骨を貫通して脳内に入った脳内異物に関しては多くの報告があるが，脳動脈内の異物に関する報告は少い．著者らは狩猟中の事故により，散弾が総頸動脈壁内に留まり，それをレ線透視下で摘出しようとして頸部を動かしたときときに頸動脈内に入り込み，上行して中大脳動脈主幹部に留まり，その後角回動脈と後側頭動脈を支配する中大脳動脈の主枝に嵌入したまれな1例を経験したので報告する．

A rare case of the shotgun pellet embolus of the left middie cerebral artery via the common carotid artery is reported.

While hunting, a 27-year-old right-handed man was shot accidentally at a distance in the left side of the neck and the right thigh. Plain X-ray films of the neck revealed a shotgun pellet at the region of the left common carotid artery (Fig. 1). A shotgun pellet of the neck entered into the lumen of the common carotid artery while its surgical removal was being performed under fluoroscopic control at Sato Clinic. He was transferred soon to the Takeda General Hospital.
