視覚性注意障害

Visual inattention.

I．はじめに

　視覚性注意障害visual inattentionなる言葉が文献に登場するのはおそらくHolmcs（1918a）が最初であろう。彼は初めdisturba nce of visual attentionという表現で，これを「視野に明らかな視覚性感覚の低下や制限がないのに，患者が患側の同名視野内の対象を認知しない状態」と定義して症例を呈示した。例示されている症例は右頭頂葉の銃創例で，視力は両眼とも6／6。視野は白色指標に対しても，色彩指標に対しても正常。形態視も視覚性定位能力も両視野で正常であった。しかし，験者が両手を患者の左右視野にかかげて指の動きを判定させると，右視野では問題がなかったが，左視野ではその動きを見落とすことがあった。また，同時に両側の指を動かすと，左側の動きをほとんど必ず見落とした。左側の刺激の見落しは対象の大きさには関係なく，右視野には1×4cmの白紙，左視野には10×7.5cmの白紙をかかげても同時に動かすと左の動きは見落とした。しかし，左側が動くことを告げて左視野への注意を喚起しておくと，左の動きの知覚は改善した。

　Holmesはこの障害をHeadとHolmes（1911）が既に記載していた皮膚感覚のlocal attentionの障害と同じ性質のものだとし，注意attentionが障害領域へ向けられないのが本態であり，この注意障害は別の領域の別の刺激が注意を要求する場合により顕著になると述べている。すなわちここでは，後にいう両側同時刺激に対する消去現象extinctionと軽度の左一側性空間無視unilateral spatial neglectが視覚性注意障害の内容をなしている。さらにHolmes（1918b）は別の論文で，visualinattentionの症状に「一度に1つのものしか見えない」という訴えを加え，この「1つ」は大きさには関係なく，大きいものであっても，まとまりあるものは1つの全体として見ることができる，と述べている。たとえば彼の患者の一人は紙の上に大きい四角を描いたものを見せられると，ただちにそれが四角であることを認知した。ところがその四角の中央に十字形を描き加えたものを見せると，今度は十字を見るが周囲の四角の認知には困難を示した（Holmes，1919）。

　In this paper we attempted to summarize the core features of so called "visual inattention". In the modern usage, the term is broadly applied to one or both of the following symptoms, i.e., unilateral hemi-inattention and extreme narrowing of attention (De Renzi, 1982). We applied the term "visual inattention" only to the latter condition.

　Visual inattention is composed of two related but distinct disorders. One is the inability to see plural objects at a time (Balint, 1909). The size of an object is not a limiting factor. An object can be perceived regardless of its size if it has a meaningful unity. Even when two items are perceived at the first glance, one of them quickly fades from the vision. Directional cue is effective in shifting attention. The other is the inability to shift attention covertly (Seelenlahmung des Schauens, Balint). This has to be distinguished from difficulty of ocular control. This lack of attentional control results in peculiar loss of spontaneity in looking.
