視床と注意障害（Thalamic neglect）

I．はじめに

　Neglect syndromeとは，Heilman^21）によると，「さまざまな刺激に対する反応や刺激にさいして要素的な感覚，運動障害をもたないのに，大脳病巣の反対側に与えられた刺激に気づかず，反応しない現象」であり，複数の感覚modalityに現われる現象としている。普通，右半球損傷に伴って左側の無視として現われ，左半球損傷に伴う右半側の無視は，頻度も少なく，程度も軽いといわれている。病巣としては，Brain以来，頭頂葉病変^5,6,19,31）が重視されているが，前頭葉病変^11,20,48），外側型脳出血^18,25,28），内包病変^15），視床病変^{7,26,27,51,52）}によるneglectも報告されてきている。

　このneglect syndromeを生じるメカニズムとして，最近，注意障害説が有力となってきている。ここでは，視床損傷とneglcct syndromeの関係について自験例をまじえ報告したいと思う。

　Unilateral Neglect is a symptom that a patient neglects half of his outer space. When a patient with neglect is asked to perform a variety of behavioral tasks in space, he neglects the hemispace contralateral to his lesion in the brain. Many investigators have noted that neglect is commonly associated with a parietal lobe lesion. However, recently neglect accompanied by lesions in the frontal lobe, basal ganglia, and thalamus has been reported.

　Only several reports of neglect after thalamic hemorrhage or infarction have been published.However, not all cases with thalamic lesion produce neglect and whether thalamic damage perse is a cause of neglect is a controversial issue.
