高血圧性脳出血の心電図に関する臨床ならびに実験的研究

Clinical and Experimental Studies on E.C.G. in Hypertensive Intracerebral Hemorrhage

Ⅰ．緒言

　脳に対する侵襲が心電図変化をひきおこすことについての観察はかなり古くから行なわれている。即ち，1922年Lennox et al^1）は臨床上，頭部や頸部の手術時には，他部の手術時に比し，心電図が変化し易いと述べ，実験的には1930年Beattie et al^2）は脳幹部の電気刺激による不整脈の発現をみている。しかし近来最も関心がよせられているのは脳卒中の心電図変化で，これについてはかなり多くの報告がなされているが，脳卒中症例における心電図変化には数多くの因子が関与しているので，その変化が真に中枢性のものであると断定するには，未だ決定的な証明が不足している状態である。また現在報告されている脳卒中症例の多くは，クモ膜下出血症例であり，高血圧性脳出血症例のものは少なく，とくに脳内血腫形成部位と心電図変化との関連についての報告はきわめて少ない。

1) Investigation of electrocardiograms of 68 pa-tients in an acute stage of hypertensive intracere-bral hemorrhage revealed that ST. T change, leng-thened QT, amplified U-waves and extrasystole were extraordinarily frequent comparing with the control. These phenomena, however, had no relation to serum electrolyte, anemia and serum protein, or autopsy findings.

Changes on E.C.G. were different and particular depending upon the time of taking the E.C.G., cerebrospinal fluid pressure and volume and loca-tion of hematoma.
