末梢神経および自律神経障害を主徴とした家族性原発性amyloid沈着症の病理学的研究—2剖検例を中心とした文献的考察

Two Autopsy Cases of Familial Polyneuritic Amyloidosis

I．はじめに

　Amyloid沈着症は全身の臓器あるいは組織にamyloidが沈着する原因不明の代謝性疾患であり，原発性amyloid沈着症および続発性amyloid沈着症に大別されている。Amyloid沈着症において，一般臓器におけるamyloidの沈着部位に関しては原発性amyloid沈着症および続発性amyloid沈着症との間に差は認められないが，末梢神経系および自律神経系へのamyloid沈着は特に家族性原発性amyloid沈着症に著明であり，臨床的にはamyloid neuropathyを呈することが知られている。

　末梢神経および自律神経障害を主徴とする家族性原発性amyloid沈着症は1950年にOstertag^19）がはじめて記載したものであるが，本症の報告はその後もまれであり，Andrade^4）の詳細な報告以来，Kantarjianら^11），Rukavinaら^20），Kaufmanら^12），Schlesingerら^21），Da Silva Hortaら^6），Delankら^8），中尾ら^18），徳臣ら^23）（hereditary peripheral neuropathyとして報告。

　Two cases out of thirteen living patients with polyneuritic primary amyloidosis in a family were studied pathologically.

　They were a 47-year-old farmer and a 36-year-old house wife. The former became aware of impotence at 41 years old and the latter inclined to constipation at 30 years old. About one year or so thereafter they together complained of numbness at feet. The numbness slowly ascended with muscle atrophy and weakness to whole lower extremities.
