I．はじめに
1941年Cairnsら14）がakinetic mutism（無動性無言）を特殊な病態として提唱してから35年を経た。この間治療法の進歩に伴い，重篤な神経系の障害により外界に対する反応が著しく乏しい状態のまま長期間生存する患者が増えつつある。一見覚醒しているようにみえながら，自発的言動がなく，外からのはたらきかけに反応しない特殊な状態に無動性無言，失外套症候群，coma vigile，locked-in症候群などがあるが，これらのうち前3者は異った観点から独立にその概念が提唱され，現在もなお相互の関係について一般的な同意が得られていない。
本稿はakinetic mutismについてその概念が提唱されたゆえんと，その後の報告をまとめ，本症候群の病態生理および症候学的位置づけを行ない，現在の問題点を明らかにしようとするものである。
The present paper is concerned with the lesions and pathophysiology of akinetic mutism. Review of the literature and three personal cases presented in an original paper are sources of this study which has been aimed to differentiate various forms of akinetic mutism ever reported, on basis of essential anatomical lesions.
The apallic syndrome and locked-in syndrome, both are similar conditions clinically, are also mentioned.
