中枢神経系の発達期における障害の臨床像について—Das Apallische SyndromとAkinetic Mutismとの関係について

I．はじめに

　特異な意識状態として，1940年Kretschmer^7）がDas apallische Syndromの概念を提起して以来，Cairnsら^1,2）のakinetic mutismをはじめ，parasomnia^4），hypersomnia^3），さらに近年Plum and Posnerの"locked-in" syndrome^17），あるいはpersistent vegetative state^5）などが提唱され，その臨床像，神経生理学的所見，あるいは神経病理学的所見の報告，さらにその異同性など^{21〜23,26,27）}枚挙に遑なく，まさに百花撩乱の観がある。

　一方，わが国では，Kretschmerに先立って，1939年に島崎^19）は「人間的分化機能の喪失を来たし癲癇痙攣性脳損傷例」を報告，この特異な意識状態を注目し論じていることは，はなはだ興味があり，深い感銘を受ける。

　Neuro-psychiatry on severely brain-damaged children has not been assured. Those patients suffer from destruction of the brain in the prenatal, perinatal or early postnatal period by some processes.

　The clinical picture of them is as follows. All of them are continously bedridden. They have no speech but a moan or groan. Primitive reactions of pleasure or unpleasure are observed.
