Angiographic criteria of moyamoya disease

はじめに

　もやもや病（脳底部異常血管網症，ウィリス環閉塞症）が一つの独立疾患であるのか，あるいは一症候群にすぎないのかという疑問は，本症発見以来常に討論されてきておりながら，いまだ決着をみない重要な問題である。

　その理由はいろいろあると思われるが，まとめてみると次の三つに大別することができよう。まず第一にもやもや病一以下本症と呼ぶことにするが一は，脳血管写により初めて発見されたものであり，その診断は脳血管写所見のみが唯一のきめ手であって，その他の臨床検査所見や病理所見からは，診断を左右するような何ものも得られず，病因が不明であることであり，第二には，本症以外の原因の明らかな後天性脳血管閉塞あるいは先天性奇形性脳血管異常と考えられるものが，時に本症類似の脳血管写所見を呈することがあり，脳血管写上においてさえ，しばしばクライテリアの混乱がみられることである。

Whether the "Moyamoya disease" is a disease entity or only a syndrome has not been determined yet. The definition and criteria of the Moyamoya disease was originally established by the charac-teristic findings of its cerebral angiogram, and no special cause or laboratory findings have not been observed. Though the angiographic findings of Moyamoya disease are very much characteristic, other cerebrovascular occlusive or malformative diseases with similar angiograms to the Moyamoya disease are sometimes mistaken and misdiagnosed as if they were the true Moyamoya disease in literature.
