Parklnson病に対して選択されている脳深部刺激療法（DBS）の刺激部位は，視床腹中間核（Vim），淡蒼球内節（GPi），および視床下核（STN）である。DBSでは，刺激を中止することによって，その効果を評価するための対照（control）を得ることができる。この方法（off-stimulation vs on-stimulation）により，DBSの効果をUnified Parkinson's Disease Rating Scale（UPDRS）^9）で評価している報告をまとめると，levodopaに反応するParkinson病のうち，（1）薬物療法のみでは制御の困難な振戦（とおそらく強剛），（2）levodopaの投与による著明なon-off（wearing-off）現象，（3）levodopa-induced dyskinesia，（4）副作用のため薬物療法に制限のある場合に，DBSの適応があると考えられる。またlevodopaに反応しなくなった進行例では，DBSの適応に限界があると推定される。

During the last decade, it has become increasingly clear that DBS represents a useful adjunct for therapies to control various symptoms of Parkinson's disease. The stimulation sites include the thalamic nucleus ventralis intermedius (Vim), globus pallidus internus (GPi) and subthalamic nucleus (STN). The clinical data of DBS therapy currently available from the literature, together with our own experience, are reviewed. The results of our double blinded evaluation of the effects of GPi stimulation (n=7) and STN stimulation (n=11) are also summarized.
