はじめに

　脳深部に埋め込んだ電極でヒトの中枢神経系を電気刺激し，その機能を制御しようとする試みの歴史は古い。著者自身もこのような脳深部刺激（deep brainstimulation：DBS）療法の研究を始めてすでに20年になる。それは長い試行錯誤の歴史であったが，近年になっていくつかの疾病の治療にめざましい成果をおさめるようになった。

　その発達の過程は刺激システムの進歩と不可分である。種々の疾患におけるDBSの効果についてはすでに別に紹介しているので，この論文ではできるだけ重複を避け，DBSのための刺激システムに重点を置いて，その歴史と現状を概観したい。

During the last decade, it has become increasingly clear that deep brain stimulation (DBS) is useful for controlling various neurological disorders, including intractable pain and involuntary movements. The de-velopment of an implantable and programmable pulse-generator has greatly facilitated the clinical application of DBS therapy. The history and clinical data of DBS therapy currently available from the literature, to-gether with our own cases treated by DBS, are re-viewed.
