MAOB阻害薬であるセレギリンとCOMT阻害薬であるentacaponeとtolcaponeの臨床試験結果を内外の二重盲検比較試験を中心に検討した。セレギリンのパーキンソン病進行抑制効果に関しては症候的治療効果があり臨床試験では否定的である。初期パーキンソン病では，レボドパ投与開始を遅らせることが可能であり，レボドパとの併用でレボドパ使用屋を減量できる。セレギリンはレボドパの効果を約30分延長し，進行期パーキンソン病のmotor fluctuationを改善し，Earlymorning dystoniaやakinesiaの改善効果を示す。長期使用時の安全性は高いと理解されている。Entacaponeとtolcaponeは血液レボドパ代謝を改善しmotor nuctuationを改善するのに有用である。セレギリンとCOMT阻害薬の併用は現段階ではとくに問題点は指摘されていない。

Clinical trials of selegiline, a type B monoamine oxidase (MAOB) inhibitor and catechol-O-methyltransferase (COMT) inhibitors are reviewed from the standpoint of Evidence Based Medicine. They are adjunctive drugs in the treatment of Parkinson's disease. This review mainly aims to analysis class 1 level clinical trials according to Evidence Based Medicine. Selegiline (deprenyl) has a long history of using in treating Parkinson's disease and is the only MAOB inhibitor in the world.
