老化が脳循環代謝に及ぼす影響に関して,これまで報告された研究を概観し,いくつかの問題点について考察を行った。老化それ自体によって,脳酸素代謝はある程度低下すると考えられる。しかし,脳血流と脳ブドウ糖代謝については未だ一定した結論に達していない。これは測定手段の精度が不十分なためと考えられる。加齢に伴う脳酸素代謝の低下は直線的ではなく,20代から50代まで緩やかに低下してそれ以降は余り変化しない。また,加齢に伴う代謝の変化は脳萎縮とは相関しない。形態学的所見と機能的所見から,脳の老化過程は単純な機能低下ではなく,代償過程の働くダイナミックなプロセスであることが明らかとなってきた。今後,測定精度を向上し,縦断的研究を行うことが望まれる。
A half century history of the studies regarding aging effect on human cerebral circulation and metabolism was reviewed and several related issues were discussed. Cerebral oxygen metabolism is expected to decline along with aging. No consistent results were reported for the aging effect on cerebral blood flow and cerebral glucose metabolism. The decline of cerebral oxygen metabolism with aging is not linear, but declining from twenties to fifties slowly, then remains relatively stable to eighties. The metabolic change with aging is discrepant with cerebral atrophic change. Morphological and functional evidences revealed that the aging of the brain is not monotonous slowing-down process, but a dynamic process recruiting compensatory mechanisms. The technical variance of the measurements of cerebral circulation and metabolism needs to be improved, and the longitudinal study design will be suitable for observing the aging effect on brain per se.
