癌性疼痛の機序について述べた。癌性疼痛には侵害受容性疼痛，神経因性疼痛，および心因性疼痛の三つの疼痛機序が混在している。また，臨床的には癌そのものによる疼痛，癌に起因する生理学的・生化学的変化による疼痛，および癌治療により発生する疼痛が考えられる。癌性疼痛の治療にはこれらの機序を念頭において治療すべきである。モルヒネが効きにくい痛みとして神経因性疼痛の存在があり，この疼痛が混在するとその疼痛機序は複雑になるため治療に難渋する。そのような場合，疼痛機序の鑑別診断の一助として有用と考えられる薬理学的疼痛機序鑑別試験（Drug challenge test）についても述べた。

Mechanisms of cancer pain is discussed in this article. Cancer pain is thought to include three essential pain mechanisms such as noxious, neuropathic and psychological pain mechanism. In the clinical field of cancer pain management, cancer pain is classified into next three categories such as pain caused by cancer mass itself, physiological and biochemical pain associated with malignant tumors and pain caused by the therapies of cancers. Therefore, management of cancer pain should include the strategies which are suitable to each pain mechanism. A pharmacological procedure to diagnose the mechanism of pain, Drug Challenge Test, is thought to be useful in the differential diagnosis of cancer pain.
