Mechanism and clinical pictures of CRPS.

身体の一定の領域で感ずる慢性痛で，その程度の痛みを起こす原因としては普通は考えられないようなある契機をもって出現する痛みを，従来，交感神経が関与しているとして反射性交感神経性異栄養症，またこのような痛みで神経損傷後に発生するものをカウザルギアと呼んできた。しかし，これらの痛みには交感神経が直接関与しているものもあればそうでないもののあることが従来から指摘されていた。したがって国際疼痛学会ではこのような部分的な痛みの再分類を行い，complex regional pain syndrome（複合部分的疼痛症候群）という概念を導人し，その第I型を反射性交感神経性異栄養症とし，第II型をカウザルギアとすることにした。しかし，これは基本的に症状による分類であり，将来これらの痛み症候群の病理学が明らかになればこの分類も再検討されるであろう。本文ではこのような状態の臨床像と治療について実際の症例を提示して解説した。

A type of severe chronic regional pain which can not be explained by the apparent causative lesion was considered to be related to the activity of the sympathetic nervous system and this type of pain syndrome has been designated as reflex sympathetic dystrophy, and the similar pain associated with the nerve damage as causalgia. It has been suggested, however, that the sympathetic nervous system is not always directly involved in these pain syndromes. Therefore, IASP coined a more general term to describe these pain syndromes, i.e. complex regional pain syndrome (CRPS).
