腰痛は臨床の場において頻繁に対応しなければならない愁訴であるがその原因はきわめて多岐にわたるために原因を明確になし得ないことが多い。それだけに治療方法の選択にも混乱があるのが現状である。現在，まだ議論の余地は残されているものの痛みの発信源がある程度論じられている腰痛は椎間板性，椎間関節性，筋々膜性，神経組織圧迫によるもの，などである。最近の話題は，障害あるいは変性に陥った椎間板組織から各種の炎症，疼痛メディエータとしてのサイトカインが証明され，炎症あるいは化学的要因が腰痛，とくに神経組織圧迫に起因する愁訴発生に大きく関与していることが証明されたことである。

　当然のことながら上記の原因の他に椎骨，骨膜の外傷，腫瘍，感染症などによる疼痛，骨盤内臓器疾患の関連痛なども考慮に入れておく必要がある。また，腰痛は関連痛，精神的，経済的加重などによって修飾されさらに複雑な愁訴を形成するので注意が肝要である。診断にあたってはまず，原因を追究することから開始すべきであるが画像診断が必ずしも愁訴の根拠とはなり得ないことも理解しておかねばならない。

Low back pain is one of the most frequently encountered complaints for an orthopedic surgeon in his or her clinic, although the etiology of this symptom complex is so multifactorial and poorly defined that precise diagnosis and treatment is not always possible. Accordingly, there are controversies in the selection of the method of treatment. The etiology of the low back pain is better discussed in patients with discogenic, facetogenic, myofasciogenic pain and pain caused by compressed neural tissues, although, the issues about the true pain generator associated with degenerative disc is still remain as quite controversial.
