片頭痛のメカニズム

Mechanism of migraine.

片頭痛における疼痛発現のメカニズムには神経，血管をはじめ環境因子，素因（遺伝的因子）など多くの因子が関わりあっている。内因性pain control systemは片頭痛発作時の疼痛の制御や閾値の調節に関与していると考えられている。片頭痛発作発症のメカニズムでは三叉神経，脳血管などを考慮に人れたtrigeminovascular theoryが中核をなしているが，serotonin. substance P，calcitonin gene related peptide，catecholamineなどの神経伝達物質が複雑に関わりあって片頭痛発作を起こすものと考えられている。分子生物学的手法の発達に伴い，片頭痛患者において原因遺伝子の検索が行われるようになってきた。家族性片麻痺性片頭痛という特殊な片頭痛では原因遺伝子として，P/Q type Ca_2＋channelの遺伝子異常が関与していることが明らかとなっている。さらに，ミトコンドリア遺伝子などの関与も考えられて検索が行われつつある。

Migraine is a common neurological disease of two main types: migraine with aura and migraine without aura. The transmission of impulses in migraine pain is modulated by the endogenous pain control system which descends from the periaqueductal gray matter (PAG). Neurotransmitters involved in the pain pathway include serotonin and peptides, such as enkephalins, substance P and calcitonin generelated peptide (CGRP).
