Change of the lacunar hypothesis

はじめに

　「脳深部の小梗塞，すなわちラクナは特定の臨床症候，すなわちラクナ症候群と対応する」というのがラクナ仮説である。この仮説の成立と検証の経過，問題点と誤まりの指摘がラクナ仮説の変遷である^1）。

　ラクナは，本来，脳内の小穴，くぼみといったマクロ病理解剖の形態をあらわす用語であり，必ずしも小梗塞とイコールではなかった。しかし，臨床的にはラクナが小梗塞と同義であることに意義があり，これが歴史的にも，小梗塞対大梗塞，穿通枝系梗塞対皮質枝系梗塞，ラクナ梗塞対血栓性ないし塞栓性梗塞という脳梗塞における対立した重要な概念の一端を背負わされてきた^2-13）。しかし，CTやMRI診断の進歩に伴い脳深部の小梗塞とラクナ症候群とは必ずしも対応せず，また両者ともその成因は一様ではないことが明らかにされてきた^1,14-17）。たとえば，内頸動脈閉塞の血行力学的虚血による脳深部小梗塞や脳出血によるラクナ症候群などがしばしば存在することが明らかとなった。

Lacunar infarctions were initially described by French literature of the mid-to-late-19th century. These were named 'lacunes', the French term for lakes. Lacunes are small (diameter, 0.5 to 15mm) focus of infarction in a deep cerebral area. A number of lacunes are seen in the brains of many elderly patients with hypertensive small artery disease and cerebral arteriosclerosis, particularly in the basal ganglia, thalamus, base of the pons and white matter.
