Catecholamine histofluorescence and enzyme histochemistry of MAO in the biopsied sural nerves, with special reference to acute idiopathic pandysautonomia. Histochemical findings of intra-neuronal catecholamine and monoamine oxidase (MAO)-containing unmyelinated axons were investigated in 5 biopsied specimens of 3 cases of acute idiopathic pandysauto-nomia (AIPD), referring to the findings of sural nerves in pastly reported AIPD. Histochemical demon-stration of catecholamine in sural nerves was done with wet-histochemical procedure using glyoxylic acid.Histochemical MAO-staining demonstrated by a new coupled peroxidation method (Kishimoto et al.; modified) was applied to biopsied sural nerve specimens at the electron microscopic level. MAO-contain-ing unmyelinated axons have been assumed to be postganglionic sympathetic ones (Matsubayashi et al.). 2 cases were examined both at extreme stage and at two or three years after the onset.

As the results, 2 of 3 cases of AIPD at extreme stage showed mild loss of unmyelinated fibers with relatively increased ratio of MAO-containing ones, and showed moderately or highly enhanced fluores-cence of catecholamine in the biopsied sural nerves. 1 of 3 cases of AIPD showed remarkable loss of unmyelinated axons including MAO-containing ones, and also showed no apparent fluorescence of catecholamine in the sural nerves at extreme stage. The follow-up study findings of sural nerve of 2 cases of AIPD disclosed a small amount of increase of unmyelinated axons two or three years after the onset.
