三叉神経の形態学的問題点：頭部問題の観点から

Morphological problems concerning the trigeminal nerve with special reference to the “head problem”.

はじめに

　三叉神経は，顔面，舌咽，迷走，（副）神経とともに鰓弓神経の一つに数えられる。鰓弓神経は，腸管の吻側端で特殊に分化した鰓弓領域を支配し，臓性線維に富む。しかしながら，鰓弓領域の吻側端，すなわち体性器官と臓性器官の移行部を支配する三叉神経には，ほかの鰓弓神経とは異なり，脊髄神経との類似点も多い。

　三叉神経の一次求心性線維の大部分は，顔面の皮膚と鼻腔口腔粘膜を支配しているが，これらは一般体性感覚系に含められる。中枢の構築も同様で，三叉神経主知覚核からの二次線維が，対側に渡って内側毛帯とともに上行することから，Lewandowsky（1904）とWallenberg（1905）は主知覚核を後索核と等価と考えた。また三叉神経脊髄路核のうち，尾側核は脊髄後柱頭部と共通の層構造をもつ（Olszewski，1950）。遠心性線維の方は臓性に属するものの，平滑筋や腺を支配する一般臓性運動系はなく，横紋筋を支配する特殊臓性運動系のみである。したがって三叉神経を理解するためには，三叉神経と他の鰓弓神経，三叉神経と脊髄神経の共通点と相違点を整理しておくことが必要である。三叉神経固有の様々な問題点については，本号のほかの総説で詳しく触れられることと思うので，ここでは，他の鰓弓神経，ならびに脊髄神経との比較において，三叉神経を概説する。

The trigeminal nerve innervates the transitional region between the somatic and the visceral organs. Although it is classified as a branchial nerve, its major fiber components are, like the spinal dorsal roots, somatic afferents. In contrast to the other branchial nerves, it lacks general visceral efferent fibers. The trigeminal nerve has both branchial nerve-like and spinal nerve-like characteristics. The organization of these two series of segmental nerves and their target organs has been under discussion since the beginning of the 19th century, and the controversy is known as the “head problem.”
