はじめに
脊髄実質中に数髄節以上にわたる空洞が生じる病態は，すでに16世紀に記載があり，19世紀のはじめには，syringomyeliaという病名で報告されているといわれている（Foster and Hudgson，1973a参照）。その後，脊髄空洞症は種々の疾患，すなわち脊髄腫瘍，脊髄外傷，後頭蓋窩および大後頭孔近傍の奇形などに随伴して認められることが知られてきた。
近年になって，画像診断法，特にMRIの発達普及に伴い脊髄空洞症の診断は，比較的容易になり，とりわけ脳圧亢進を伴わない両側小脳扁桃の下垂，すなわちChiari I型奇形に伴った脊髄空洞症の頻度は，今まで考えられてきた以上に高いことが明らかになってきた（Aubin，et al.，1981；Sherman，et al.，1986；Aubin，et al.，1987；Li，et al.，1987；井須ら，1990）。このChiari I型奇形を伴った脊髄空洞症は，外科的治療の対象となり，良好な成績も得られることから，最近特に臨床上注目を集めている（Batzorf，et al.，1988；井須ら，1988；Isu，et al.，1990；山浦，1991）。
Recent advances in imaging techniques such as computerized tomographic scanning and magnetic resonance imaging have greatly facilitated the diagnosis of syryingomyelia and associated abnormalities. There have been a large number of reports on the neuropathology of syringomyelia, but few of them have described the morphological features of the disease. In the present study we investigated in detail the neuropathological features in 18 autopsy cases of syringomyelia, and discussed the pathogenesis of this condition.
On the basis of the pathological findings, most of the 18 cases were classifiable into three groups:
