はじめに

　脊髄腫瘍とは脊髄および脊髄神経根に圧迫を加え，種種の神経症状を発現する腫瘍性病変である。脊髄，神経根はもとより，脊柱管内の組織（硬膜，くも膜など），脊椎骨などいずれからも発生する。他部位からの転移性腫瘍も含まれる。

　脊髄腫瘍は発生部位により髄内腫瘍，硬膜内髄外腫瘍，硬膜外腫瘍に分けて考えることができ，鑑別診断を進めるうえでも便利な分類である。ここでは各腫瘍の臨床的，病理学的特徴，画像の特徴を述べる。最近導入され高い評価を受けているMR画像所見については詳述する。

Tumors of the spinal cord include all the tumors involving the spinal cord and nerve roots, resulting in neurological symptoms and signs. They may arise from the neural tissue, from the meninges, from the sorrounding bones and soft tissues, from embryonal rests or as metastases from neoplasms in the cranial cavity or elsewhere in the body.

Spinal cord tumors are traditionally classified as intramedullary, extramedullary-intradural and extra-dural. This classification is practical and convenient to radiologists as well as to surgeons.
