臨床医が行う研究の最終目標は新規治療開発〜疾患克服であり，それを実現するためには新規治療薬の候補（シーズ）を開発し，臨床試験（治験）を経て国の承認（薬機法承認）を得，より有効性・安全性の高い治療を全国（あるいは全世界）の患者に届けるというプロセスが必要である。治験のPIとしてのキャリアパスを進むには，新規治療を開発しようとする意思と，シーズの発案から治験実施，承認まで一貫した長期ビジョンを持つことが最も重要である。本論ではシーズ開発の方法から特定臨床研究・治験の実施を効率的に実践するための道筋を，自身の医師主導治験の経験を含めて概説する。

Abstract

The final goal of clinical studies performed by physician-scientists includes the identification of novel therapeutic candidate drugs (seeds), clinical trials, and overcoming of diseases that improves patients' outcome and quality of life. To achieve this, dense intention of the development of novel treatments, and a long-term vision from the seeds to the approval of the drugs, via clinical trials, are required for primary investigators (PI). This review article explains the process from the seed development to drug approval, based on the author's experience on investigator-led clinical trials.
