抄録

　目的：イプラグリフロジンの有用性と腎機能に与える影響を検討する．

　方法：外来通院中の2型糖尿病患者27名（男性24名，女性3名）に対し，イプラグリフロジン50 mgを投与し，3カ月後の糖代謝，腎機能に与える影響を評価した．

　結果：随時血糖（mg/dL），HbA1c（%），体重，BMI（kg/m²）は経過全体として改善していた．また，尿中アルブミン/クレアチニン比（以下U-ACR）やeGFRは開始前と比較して3カ月後まで有意に低下していた．また，収縮期血圧は開始前と比較して1カ月後から有意に低下していた（p＜0.001）．アルブミン尿の改善に影響を与える因子として，U-ACRの基礎値と3カ月間のU-ACRの変化量の間には強い負の相関が認められた（相関係数＝−0.86，p＜0.001）．

　結論：2型糖尿病患者へのイプラグリフロジン投与により，糖代謝だけでなくアルブミン尿の減少が認められた．

　Aims : The objective was to investigate the metabolic effect of ipragliflozin.

　Methods : We conducted a retrospective observational study. Participants comprised 27 diabetic patients with early stage of diabetic nephropathy who were receiving 50 mg ipragliflozin. We assessed the transition of clinical indicators.

　Results : Compared to baseline, hemoglobin A1c decreased significantly after 1 month and was stable until after 12 weeks（p＜0.05）. Significant reductions in systolic blood pressure and body weight were observed at 1 month. After 3 months, urine albumin-to-creatinine ratio （U-ACR） decreased significantly（p＜0.05）.

　Conclusion : Ipragliflozin improves blood glucose, blood pressure, and albuminuria and may be therefore a useful medicine for comprehensive management of type 2 diabetes.
