　19歳の男性。左陰嚢腫大を主訴に当科を受診した。触診上，鷲卵大の左陰嚢を触れ，透光性を認めた。超音波検査では水腫と左精巣に接して3cm大の腫瘤を認めた。また腫瘍マーカーはすべて基準値内であった。陰嚢内腫瘍および陰嚢水瘤の診断で腫瘍摘除，水瘤根治術を実施した。術中，有茎性の腫瘍が精巣白膜に認められ，病理組織学的に性索／間質腫瘍，不完全分化型と診断した。異型核分裂像が認められること，また悪性症例の報告も散見されることから，のちに左高位精巣摘除術を行った。現在外来経過観察中であるが，再発，遠隔転移の所見は認めていない。

A-19-year old man was referred to our hospital in August 2001, complaining of painless swelling in the left scrotum. On exmination, the left testis revealed an enlarged elastic hard mass with positive transillumina-tion. Ultrasonography demonstrated approximately 3 cm large mass attached to the left testis. The serum tumor markers were found to be in normal range. The patient underwent a tumor resection with hydrocelectomy under the diagnosis of testicular tumor and hydrocele. On operation, we found the mass seemed to arise from the tunica albuginea.
