　腎細胞癌に対して用手補助腹腔鏡下手術（HALS：hand-assisted laparoscopic sur-gery）にて根治的腎全摘除術を行った9例と開腹手術にて行った8例の手術侵襲について比較検討した。平均手術時間と平均出血量はHALSではそれぞれ252分，176mlで，開腹術では254分，298mlであった。術後CRPは術前，術後1日目では有意差は認めなかったが，術後2日目ではHALSのほうが有意に低値であった（p＜0.05）。白血球数は術後1日目，2日目，7日目でHALSのほうが有意に低値であった（p＜0.05）。これらのことから開腹術に比べ，HALSのほうが低侵襲であることが示唆された。

We report our experience of hand-assisted laparoscopic surgery (HALS) performed in 20 patients with renal tumor, atrophic kidney and urothelial tumor. We com-pared 9 hand-assisited radical nephrectomy to 8 conven-tional radical nephrectomy. Mean operative time and estimated blood loss in the hand assisted group (252 minutes and 176 ml respectively) were not significantly different from the open surgery group (254 minutes and 298 ml respectively). However, CRP 48hrs after the operation was significantly lower in the hand assisted group than the open surgery group (p＜0.05).
