精巣上体平滑筋腫の1例

Primary Epididymal Leiomyoma:A Case Report

　52歳，男性。人間ドックにて左陰嚢内に無痛性の腫瘤を指摘され来院した。精巣上体尾部に圧痛を伴わない小豆大，表面平滑な弾性硬の腫瘤を触知した。超音波検査でも同部位に直径8mmの周辺組織との境界明瞭な腫瘤病変を認めたため，精巣上体腫瘍を疑い手術を施行した。腫瘍は表面平滑で，精巣および鞘膜との癒着は認めなかったが，悪性腫瘍を否定できなっかたため左精巣上体摘出術を施行した。組織学的には精巣上体平滑筋腫であった。

A 52-year-old-man visited our clinic with a chief complaint of painless induration in the left scrotum. A red-bean sized induration was palpated at the tail of the left epididymis, and left epididymal tumor was suspected by echography. Left epididymectomy was performed and at surgery the tumor was smooth and was found to be well-demarcated. Histological examination revealed primary epididymal leiomyoma, which is the second most common benign tumor of the epididymis. We reviewed 79 cases of this disease collected from Japanese literature and discussed its clinical features.
