　経直腸的前立腺生検の対象患者には高齢者が多く，抗凝固薬・抗血小板薬（以下，抗血栓薬）を内服しているケースは少なくない．抗血栓薬を中止することにより凝固能がリバウンドし，心血管イベント上昇のリスクがあることが指摘されている．当施設では抗血栓薬継続のまま経直腸的前立腺針生検を実施しており，この生検後の合併症について後方視的に検討した．2001年7月から2013年12月に抗血栓薬継続下に実施した115例を対象とした．25例が抗凝固薬を，99例が抗血小板薬を内服，9例が両者を併用していた．入院を要する出血性合併症は1例も発生しなかった．経直腸的前立腺生検は抗血栓薬内服を中止することなく実施可能と考えられた．

Abstract

The aim of this study was to determine whether antithrombic agent increases major complications that required hospitalizaion after transrectal ultrasound (TRUS) guided prostate biopsy (PB). Between July 2001 and June 2012, in 785 patients who underwent transrectal PB, 115 patients took antithrombic agent. They all showed hematuria, rectal bleeding and hematospermia and there were no severe bleeding complications. Of the patients on antithrombic agent, 1 (0.9%) had febrile complications, compared with 17 (2.6%) of those not taking antithrombic agent (no significant difference). There was no statistically significant difference in the incidence of hematuria or overall bleeding after biopsy between the groups. We believe there is no evidence that antithrombic agent needs to be discontinued before transrectal prostate biopsy (Rinsho Hinyokika 72 : 179-183, 2018).
